Photo/Academic Work Consent

St. Theresa School assumes your consent to use photos and/or academic work of your child in school and parish publications in order to share information about the school. School publication include, but are not limited to: the website, school yearbooks, student academic work, advertisements, annual reports, posters, newsletters, parish bulletins and other public relations materials. In additional, local news organization may hear of our activities or events and our school may invite or allot them to photograph or record our events.
In all cases, safe practice will be used including not posting names of students on web publications.
You DO NOT need to fill out this form unless you are REFUSING the assumed consent to use your child’s photo and/or academic work.
If you would prefer your child’s photo and/or academic work NOT be used, please complete the form below. If you are ok with your child’s photo and/or academic work being used, you DO NOT need to complete this form!
